What to check

Warning signs in a document

No QR or online verification: The document does not point to a public verification source that confirms its validity.
Code that cannot be verified: The certificate code does not appear in the official verification or returns no result.
Altered data: Names, scope, standard or dates show inconsistencies, retouching or mixed typefaces.
Expired date: The validity shown on the document has already expired as of today's date.

Expired and false are two different states

A certification has a defined validity period. When that period ends, the document is no longer valid: it reflects a conformity that existed and then lapsed. That describes the certificate's status; its authenticity remains.

If your doubt is only about validity, public verification shows the certificate's current status without the need to open a report.
